The thirty days after hospital

Transitional Care Management

The riskiest month. We reach you within two business days of discharge, see you in person inside a week or two, sort out the medication list, and stay reachable while things settle.

Comfort when treatment is no longer the goal

Hospice Services

Care focused entirely on comfort, dignity and presence, delivered wherever you call home. Medications, equipment and 24/7 support are included, and families are supported afterwards too.

The one people get wrong

Palliative care is not hospice

This is the single most common misunderstanding we hear, and it stops people asking for help they could have had years earlier.

Palliative care

  • Can start the day you are diagnosed
  • Runs alongside chemotherapy, dialysis or surgery
  • You keep your oncologist, cardiologist and every other specialist
  • Many people receive it for years

Hospice

  • For advanced illness, when curative treatment has stopped
  • A Medicare benefit the patient elects, not just a service
  • Medications, equipment and 24/7 support are included
  • Bereavement support continues for the family afterwards

A palliative patient can still be pursuing a cure. We manage comfort and quality of life alongside that treatment, not instead of it.

A note on hospice

Our hospice care is provided by Lenity Light Hospice

Lenity Light Hospice is a separate organisation that works closely with us. We refer patients to them for hospice care, and they refer patients to us for palliative care. If hospice is the right step, we will be clear with you about which organisation is providing which service.
